Notice of Privacy Practices
How we use, share and protect your health information, and your rights.
In Short
Chiro Mamas keeps your health information private. We use it to care for you, to run our office and to follow the law. We do not sell it and we do not use it for advertising. You can ask to see it, get a copy, fix it, limit how we share it or get a list of who has seen it. If you have a concern, call us at 725-250-2699 or file a complaint with us or with the government. You will never be treated differently for doing so.
Who This Notice Covers
This notice applies to Chiro Mamas and Dr. Justine J. Rhee, D.C., at 6675 S. Tenaya Way, Suite 200, Office 19, Las Vegas, NV 89113. It covers the health information we hold about mamas, minis and babes, including prenatal, postpartum, pediatric, teen and women’s health patients. For a child, we share this notice with the parent or legal guardian.
Our practice is self-pay. We do not bill insurance. We follow the federal privacy rule known as HIPAA as our standard, even where the law may not require it of a self-pay office, because you deserve the same protection either way. Where Nevada or California law gives you stronger protection, we follow the stronger rule.
What Health Information We Keep
We keep what you and your family share with us and what we create while caring for you: intake forms, health history, exam findings, X-rays when they are needed, care plans, visit notes, photos taken with your permission, messages, appointment records and payment records.
How We Use and Share Your Information Without Extra Permission
The law lets us use and share your information in a few everyday ways.
- Treatment. To care for you, including talking with your midwife, doula, pediatrician, OB or another provider when you ask us to or when a referral is needed.
- Payment. To take payment, send receipts and give you a superbill you can submit to your own insurance for possible reimbursement.
- Office operations. To schedule visits, send reminders, keep records, check quality of care and train our team.
- Required by law. To answer a court order, report suspected child abuse or neglect, report certain public health matters, or respond to a licensing board when the law requires it.
- To prevent serious harm. To protect you or someone else from a serious and immediate threat to health or safety.
When We Ask for Your Written Permission First
We ask for your written permission before we use or share your information for anything else. This includes marketing, testimonials, before-and-after photos, and any outside sharing not listed above. We never sell your health information. You can cancel a permission at any time by telling us in writing. Cancelling does not undo what we already shared while it was in effect.
Psychotherapy notes, reproductive health care information and similar sensitive records get extra care. We do not share information about reproductive health care for the purpose of investigating or punishing someone for seeking lawful care.
Your Rights
- See and copy your record. Ask to look at or get a copy of your chart, on paper or electronically. We answer within 30 days. We may charge a reasonable, cost-based fee for copies.
- Ask for a correction. If something in your record is wrong or incomplete, tell us and we will review it.
- Ask us to limit sharing. You may ask us not to share certain information. If you paid for a service in full out of pocket and ask us not to share it with your health plan, we will agree.
- Choose how we contact you. Tell us if you prefer text, phone, mail or a certain time of day.
- Get a list of disclosures. Ask for a list of times we shared your information for reasons other than care, payment and office operations, going back six years.
- Get a paper copy of this notice. You can have one at any time, even if you agreed to receive it online.
- Name someone to act for you. A parent, guardian or person with legal authority can use these rights for you or your child.
- Be told about a breach. If your information is ever exposed in a way the law covers, we will tell you without unreasonable delay and within 60 days.
Extra Rights in Nevada
Nevada law protects health records and gives people more say over consumer health data. Under Nevada SB 370, you may ask to confirm whether we collect your consumer health data, see it, have it deleted, and stop us from sharing it. We collect consumer health data only to care for you and to run this website, and we do not sell it. Nevada patients also keep the rights in NRS Chapter 629 on medical records. Our full website data practices are in our Privacy Policy.
Extra Rights in California
California patients and visitors are protected by the Confidentiality of Medical Information Act (CMIA), the California Consumer Privacy Act (CCPA) as amended by the CPRA, and related laws. We do not sell or share your personal information for cross-context advertising. You may ask to know, correct or delete personal information we hold, and you will not be treated worse for asking. Medical information covered by CMIA and HIPAA follows those laws first.
Patients Outside Nevada and California
If you live in another state, these rights still apply to you. If your state gives you more, we follow your state’s rule when it applies to us. We provide care in person in Las Vegas only.
How We Protect Your Information
We keep records in secure systems, limit who can see them to people who need them, use passwords and encryption where available, and train our team on privacy. We use trusted vendors for scheduling, records, payments and secure messaging, and each one is expected to protect your information. Our use of AI tools, including office note-taking, is described in our AI-Generated Content Disclaimer.
Our Duties
The law requires us to keep your information private, give you this notice, follow it, and tell you if there is a breach. We may change this notice. The new version will apply to all information we hold, and we will post it here and keep a copy in the office.
Questions or Complaints
To ask a question, use a right above or file a complaint with us, call or write to our Privacy Officer, Dr. Justine J. Rhee, D.C., at 725-250-2699 or 6675 S. Tenaya Way, Suite 200, Office 19, Las Vegas, NV 89113. We will not retaliate against you for filing a complaint.
You may also contact the U.S. Department of Health and Human Services Office for Civil Rights at ocrportal.hhs.gov or 1-877-696-6775. Nevada residents may also contact the Nevada Attorney General’s Office, and California residents may contact the California Attorney General or the California Privacy Protection Agency.
